TITLE:                  

               SCHOOL SOCIAL WORKER

 

POSITION TYPE:

               Exempt

 

PRIMARY FUNCTION:

               Provides assessment, counseling, and consultative services that support positive academic and social outcomes

               for students.

 

MINIMUM QUALIFICATIONS:

  • Holds a Master’s Degree from an accredited school of social work. Holds a Virginia license with a school social worker endorsement.
  • Demonstrates strong oral and written communication skills. Demonstrates professional and personal characteristics necessary for working effectively with all students, parents, school personnel and other members of the community.
  • Possesses the ability to quickly learn and apply new technology.
  • Demonstrates strong organizational and management skills.
  • Demonstrates the ability to prepare and disseminate written reports within required timeless.
  • Possesses the ability to travel to move from one work location in the course of the day.
  • Possesses sufficient strength, ability, dexterity, and mobility to carry out all performance responsibilities.
  • Demonstrates ability to conduct trans-disciplinary and interagency professional interactions. Possesses the ability to assess the needs of student and families and to provide intervention and referrals to address identified needs.
  • Demonstrates a commitment to an integrated consultative service delivery model.

 

PREFERRED QUALIFICATIONS:

  • Licensed Clinical Social Worker preferred
  • Bi-lingual School Social Worker preferred

 

REPORTS TO:

               Director of Student Services

 

PERFORMANCE RESPONSIBILITES:

  • Completes sociological assessments by conducting parent and/or student interviews, file reviews if needed, and any checklists, as required.
  • Consults with teachers and specialists on developing and implementing classroom interventions, conducting functional behavioral assessments, and behavior intervention plans.
  • Refers students with suspected mental health concerns to community mental health assessment and service organizations. All school employees are mandated reporters of suspected child abuse or neglect.
  • Organizes time, resources, and workload in order to meet job responsibilities.
  • Provides prevention and intervention services to strengthen school and family relationships and to facilitate communication between the school and home environments.
  • Serve as a contact person for Prince George Social Services Learnfare Program. Report monthly to the eligibility worker school age truants. Develop Truancy Prevention Plan with parent and eligibility worker.
  • Provide follow-up for referrals to community resources.
  • Participate in Child Find activities to located unidentified students with disabilities.
  • Inform parents in need of resources available for their school-age children.
  • Serves as a contributing assessment-based member in Special Education Eligibility Committee.
  • Participate as a contributing member of the Manifestation Determination Review team.
  • Engages in professional growth and development to maintain licensure and adherers to The National Association of Social Workers Code of Ethics.
  • Provides training and workshops for students, parents, school personnel, and community members.
  • Conducts individual and group crisis intervention or short-term counseling. Completes threat assessments as requested.
  • Participates as part of the school’s crisis management team and provides support to schools and families in the event of a crisis as requested.
  • Participates as part of a school-based multi-disciplinary teams to coordinate and implement school, family, and community supports for student demonstrating emotional and behavior difficulties in school.
  • Consults with teachers, parents, students, and outside agency personnel concerning student needs.
  • Attends Family Assessment and Planning Team (FAPT) meetings as required and communicates meeting outcomes to the Director of Student Services.
  • Complete required FAPT paperwork for identified students as required (initial, annual, discharge, etc.). This FAPT paperwork includes, but not limited to: CSA Consent to Exchange Form, CSA Referral Form, IFSP (Individualized Family Service Plan), and FAPT SPED Utilization Review.
  • Complete required financial FAPT paperwork and duties for identified students as required (initial, annual, discharge, etc.). This required financial paperwork and duties includes but is not limited to: Creating initial Budget Encumbrance form, disseminating CSA purchase of service orders, processing monthly invoices, and facilitating securement of placement agreements to include rates/tiers of service.
  • Assist homeless families with school age children in securing resources for successful school attendance.
  • Serve as a case manager and/or member of the Prince George Family Assessment and Planning Team. Refer families to PG FAPT for assistance, secure private day school placements for students in need, and monitor educational and social-emotional progress. Address mitigating factors for student’s educational and social-emotional needs.
  • Maintain annual CANS certification and complete CANS assessments for identified students, as required (initial, annual, discharge, etc.)
  • Refers families to appropriate community resources to assist them in caring for their children.
  • Consult with teachers on developing and implementing classroom interventions.
  • Maintain accurate, complete records and reports as required by law, division procedures, and administrative regulation
  • Participate and facilitate team-based Functional Behavioral Assessments and Behavioral Intervention Plans, as needed.
  • Provide relevant professional development to school staff, as needed.
  • Perform other related duties as assigned by supervisor.

 

TERMS OF EMPLOYMENT

               Salary based upon teacher’s scale with extended days

 

EVALUATION:

               Performance of this job will be evaluated in accordance with provisions of the Board’s policy on Evaluation of

               Personnel.
